Our comprehensive dental program provided by Teamsters Health and Welfare gives you and your eligible dependents coverage for cleanings, checkups, and a wide range of dental services to keep your smile in top shape. Coverage is available for both in-network and out-of-network. The plans have set allowances for all covered dental procedures.

Annual Maximum $3,000 per person
Oral Surgery/Extractions $25 copay per tooth
Endodontic surgery (Root Canal) $25 copay per tooth
Periodontal surgery $25 copay per quadrant
Crowns, bridges, etc. $30 copay per tooth
Partial/full dentures $50 copay per unit
Orthodontics (ages 10 yrs-18 yrs) $100 copay per case (once per lifetime)
Cone Beam CT, Implant Abutment, Implant Crown Partial Reimbursement based on balance of annual maximum left at the end of the year

Out-of-Network providers have a set fee schedule, and there may be a balance bill Review the out-of-network plan allowances here. For the most affordable dental, be sure to use in-network providers whenever possible.
